Geriatric Program

If you are an older adult with complex health needs, you may be referred by your family doctor for a geriatric assessment. The Geriatric Program’s interdisciplinary team provides specialized care and support to families that can help address health issues affecting the frail elderly.

Do I need a geriatric assessment?
You may benefit from an individualized geriatric assessment if you are experiencing difficulties with:

  • memory,
  • depression,
  • mobility and gait,
  • daily care (bathing, grooming, or cooking), or
  • medication management.

How will I get a geriatric assessment?
Once you are referred by your family doctor, a geriatric assessment can be provided to you in the setting most appropriate to your needs – at home; in a long-term care, community care, or acute care facility; or at the Provincial Geriatric Program office.

Who is part of the geriatric team?
The geriatric team includes:

  • geriatricians,
  • a nurse practitioner,
  • a geriatric nurse specialist, and
  • an administrative support.
