Living a Healthy Life - Self Management Program

Living a Healthy Life is a Chronic Disease Self-Management Program for people with any type of chronic disease, their caregivers and loved ones, and anyone ready to make lifestyle changes to improve their health.


What will I learn?
Program participants will learn how to get started with healthy eating and exercise, manage symptoms, communicate better and make daily tasks easier.


How long is the program?
Living a Healthy Life is a six week program. Sessions are held once a week for 2½ hours and are available in English or French.

May 23 to June 27 (Tuesday afternoons)
1:30 to 4:00 PM

Montague

May 4 to June 8  (Thursday evenings)
6:00 to 8:30 PM

Charlottetown
May 3 to June 7 (Wednesday mornings)
9:00 to 11:30 AM
